Custom HTML Themes?

Anyone have a recommendation for getting a custom HTML theme?

I'm looking for something similar to this site... hxxp://www.carinsurancelist.com/ (Not my site or niche by the way) with similar functionality (lead generation)

I dont mind paying for one if the price is reasonable. Doesnt have to be original either, I'd be fine buying one off a Template Monster type site too but haven't been able to find a good one on the sites i know of.

That looks so simple, that if you find something that is even close, you could hire a freelancer to do the editing and get it done, or maybe even ask what it would cost to make it from scratch... Just look for a website designer from places like elance, odesk etc..
